TRANSPORTATION/ TEMPORARY GUARDIAN CONTRACT
By signing this contract, the parties involved are entering into an agreement, in which the owner still retains full and complete ownership of the bird, birds or animal involved. Ownership belongs to the owner, not the caretaker or person who is providing the service.
By entering into this contract, the caretaker/provider is only given limited custody of the parrot/parrots in question.
Provider: ________________________________is held responsible to either transport or care for (parrot's name/s),____________________________ & band #_____________and will return said parrots on:

Caretaker/provider ____________________________________is fully responsible to return said parrots to the owner or face possible criminal charges.
The caretaker also has permission to take parrot for veterinary care if the parrot becomes ill or injured while in their care. Caretaker must provide the safest means of transportation possible. Caretaker shall not be held responsible for any veterinary costs, or death, if said parrot dies while in their care, unless illness or death is due to negligence on the part of the caretaker.
